Parent and Player
Code of Conduct Agreement
The following agreement is made between Cincy Swish, its players and their parents.
Responsibilities of the Player:
Players will show respect toward referees, opposing players, coaches and fans. They will not harass, abuse, or be rate a referee for any reason.
Players shall refrain from using profanity during games or practices.
Players shall not participate in physical violence or threats thereofduring games or practice.
Attendance at all practices and games is mandatory. Coaches must be notified prior to a missed practice.
Make up sessions must be pre-approved by the teanfs head coach.
Players are expected to show respect for all coaches and adhere to all instructions.
Players shall bc on time to all practices and games.
Players maintain the highest standard of conduct and good sportsmanship.
Players are expected to wear an approved Swish uniform during all games and Swish functions.
Use or possession of any illegal drug substance or illegal activity ofany kind will not be tolerated.
Responsibilites of the Parent/Spectator:
Parents must set the example by exmplilying the highest standards of sportsmanship.
Parents participate in a game by watching, cheering and supporting the efforts of all participants.
Parents must understand that the game is for the players. their participation and enjoyment is the most important element.
Parents shall maintain a professional relationship with their child's coach and avoid a confrontation immediately following the game.
Parents must never criticize a coach in a public manner.
Parents must exhibit good sportsmanship and refrain from loul language and verbal abuse of players. coaches, referees, opponents or spectators.
Parents shall refrain from criticism during games or practices.
Parents shall not coach or otherwise instruct players during practices or games.
Parents should call the coach directly to discuss concerns or issues.
Parents are expected to have their child on time for training and games.
Parents shall stay current on all financial obligations lo Cincy Swish.
All fees, are non-refundable unless a medical condition causes a player to miss more than half the season.
Parents are encouraged to get involved with our organization and promote associated games and events in a positive way.
By typing my name in the boxes below, I recognize any responsibility to abide by the Cincy Swish Code of Conduct. l acknowledge that I have read and agree to such. Any violations of the Player and Parent Code of Conduct may be grounds for suspension or dismissal of the parent,child or both from the team. All disciplinary action is the responsibility of the Director of Coaching. Suspensions and terminations may be appealed to the Cincy Swish Board.
PLAYER LIABILITY RELEASE
l . On this day and in consideration for being accepted and allowed to participate in the activities and events associated with Cincy Swish Basketball Association, the undersigned, intending to be legally bound hereby, does hereby release and hold harmless, from any and all liability, Cincy Swish Basketball Association, and all of its employees, agents, trustees, representatives, related companies and partners/lowriers/shareholders/directors. This release is for any and all liability for personal injuries (including death) and for property loss or damage occasioned by, or in connection with, or arising out of any activity performed and undertaken in connection with activities of and related to Cincy Swish Basketball Association.
2. The undersigned hilly agrees and understands that this is a full and complete release of any and for all liability of Cincy Swish Basketball Association, for any injuries or damages arising out of any and for all activities performed and for undertaken in connection with activities of andfor related to Cincy Swish Basketball Association.
3. The undersigned gives permission to Cincy Swish Basketball Association to obtain medical assistance in the event of an emergency or medical condition (including injury). This permission includes, but is not limited to, the administration of medicines, surgical treatment, x-ray examination or hospitalization as might be ordered by a licensed medical doctor. l release and discharge Cincy Swish Basketball Association any and all liability for any first aid rendered or treatment performed pursuant to this consent.
4. The undersigned hereby acknowledges that he/she understands that basketball, and all associated activities, including drills, practices, exercises and games, can cause and contribute to physical injury and for death. The undersigned agrees and acknowledges that he/she understands the risks associated with basketball and its related activities, and hereby freely assumes any and all risks, and further releases Cincy Swish Basketball Association from all liability related to personal injury and death.
5. The undersigned voluntarily accepts any and all responsibility for his/her (or his/her child's) safety and welfare while participating in the athletic activities in connection with Cincy Swish Basketball Association, with a full understanding of the risks involved, and hereby releases and holds harmless Cincy Swish Basketball Association for any injury or claim resulting from such participation with Cincy Swish Basketball Association.
6. The undersigned further acknowledges and agrees that travel may be required in connection with participation in the Cincy Swish Basketball Association, and may include travel to and from events, games, practices and/or camps. The undersigned hereby releases from liability, and holds harmless, Cincy Swish Basketball Association from any and all injuries and damages to person or property which arises out of and is caused by any such travel.
7. I hereby grant permission to Cincy Swish Basketball Association to utilize photographs and video of me or my dependent(s) as a source for marketing the Cincy Swish Basketball Association and their programs. These photographs/videos may be posted on the Cincy Swish website, social media, used in presentations, or emails that directly relate to Cincy Swish promotions or publications. No other unauthorized use will be permitted. lt is agreed that Cincy Swish Basketball Association is not responsible for unauthorized use of copied photographs and video.
8. The undersigned agrees to take no legal action against Cincy Swish Basketball Association because of any accident or mishap or injury or death, related to the participation in the Cincy Swish Basketball Association.
9. The undersigned agrees that he/she is not under any undue influence or duress, and is not currently under the influence of any drugs or alcohol that would affect his/'her ability to hilly comprehend the contents contained in this release.
10. If this Liability Release is signed by the parent or guardian of a participant in Cincy Swish, it is understood and agreed that said parent or guardian understands and consents to all contents of this liability release, and is fully bound thereby, and hereby consents to his/her dependent's participation in Cincy Swish.
